Apply and be accepted into Providence Academy. A family cannot start the EdChoice Application process until the student has been accepted into the program.

2 STEP

SCHOOL PORTION

Submit the following documents to the school:

(1) EdChoice Initial Application or EdChoice Renewal Form

(2) Birth Certificate (only for initial applications)

(3) Recent Utility Bill (for both applications; bill must list address twice)

All names and addresses must match on Application, Birth Certificate, and Utility Bills
